What this evaluation is, and what it is not. Florida's form does not ask a doctor to treat or manage your medical condition. It asks a single functional question: whether a disability caused by a qualifying condition leaves you unable to walk 200 feet without stopping to rest. That is the question Dr. Schapiro evaluates. He does not diagnose your condition, does not treat it, does not prescribe for it, and does not replace the specialist or primary care physician who manages it.
