Our address is a business address, not a walk-in clinic. Every Provider Notes evaluation happens by live video, and you must be physically in Florida during your visit. If you would rather speak to a person before booking, call us.

Each doctor evaluates only conditions within their own Florida scope of practice. We match you to the right one based on the condition you describe, so you never have to choose.
The second list matters more than the first.
Passing the first one is not the same as passing the second.
Our questions screen for whether your situation is plausibly within Florida's criteria and within our doctors' scope. It is a screening tool. It is not a medical opinion and it is not a decision about your permit.
A Florida-licensed doctor evaluates you live, asks about your condition and your walking limits, and applies independent clinical judgment. Only the doctor can certify the form, and only the doctor decides.
Florida's criteria are set out in section 320.0848, Florida Statutes, and printed on the state's own form. You can read them yourself on the official HSMV 83039 form. A certification has to be signed within 12 months before you submit your application.
The Florida Department of Highway Safety and Motor Vehicles issues disabled parking permits, through your county tax collector or a license plate agency. Provider Notes performs the medical certification only. We are a private telehealth practice, not a government agency, not affiliated with one, and not able to issue, expedite or guarantee a permit.
After your visit you take or mail the signed form and your Florida license or ID to your county. Many counties accept it by mail. Official information is on the FLHSMV disabled person parking permits page.

Because we do not charge until 24 hours before your visit, most cancellations cost you nothing at all.

You receive a clear explanation of why. The $119 is not refunded, because the evaluation you paid for was performed. You are free to seek certification from your own physician or another certifying authority; Florida allows several types of licensed provider to sign the form, and that list is printed on the form itself.
Our doctors are paid the same amount whether they certify you or not. There is no financial incentive anywhere in this business to approve someone who does not qualify.

Provider Notes operates as a HIPAA-covered health care provider. Your booking, your visit and your signed paperwork are handled in secure systems, and vendors who handle patient health information on our behalf do so under written business associate agreements.
We do not sell patient information. We do not rent or trade it. We do not use your health information to target advertising, and we do not send health information to advertising platforms.

Caregivers and family. A family member or caregiver may complete the eligibility check and book on the patient's behalf. The patient does have to be present on the video visit, because Florida law requires the doctor to evaluate the patient directly. Caregivers are welcome to join.
If something goes wrong, tell us first. Most problems are a scheduling or paperwork issue we can fix the same day. If we cannot resolve it, you always retain the right to raise a concern about any Florida-licensed practitioner with the Florida Department of Health, or a privacy concern as described in our HIPAA Notice.
