This short check tells you whether a video visit with one of our Florida-licensed doctors is likely to be the right path for a disabled parking permit. It takes about two minutes, it is free, and it never asks for a card.

If your answers fall within Florida's criteria and within our doctors' scope of practice, you are taken to a page where you can choose an appointment time. Nothing is charged when you book. Your card is saved and charged $119 approximately 24 hours before your visit, and you can cancel or move the appointment before then at no cost at all.
The visit itself takes about five minutes by private video. If the doctor certifies you, they complete and sign the doctor's portion of Florida form HSMV 83039, or HSMV 83040 if you are an expectant mother. You then submit the signed form to your county tax collector, who issues the permit.
If the check shows your situation falls outside Florida's criteria, or outside what our two doctors can evaluate, we tell you immediately and you are charged nothing. Florida allows several types of licensed provider to sign the form, so your own physician may be able to help.

Florida sets the criteria in section 320.0848 of the Florida Statutes and prints them on the application form. In general, a permit is for a person who is certified as legally blind, or who has a qualifying condition that leaves them unable to walk 200 feet without stopping to rest. The walking limit has to be caused by one of the conditions the state lists. The doctor makes that determination during your visit.
