How it works in Clearwater

Take it to the county

If certified, you receive your signed form within 24 hours to submit to the Pinellas County Tax Collector's office or a license plate (tag) agency.

Provider Notes serves patients across Florida, including Clearwater and the Tampa Bay area. Your evaluation happens by video, so there is no office to drive to. If your doctor certifies your application, your completed Florida form is signed and returned to you, and you submit it to your local county office to receive the physical placard.

What the fee covers

The $119 pays for the doctor's clinical evaluation, not the outcome. Our doctors are paid the same whether they certify you or not, so you get an honest clinical opinion.

Certification is a clinical decision made during your visit. It is not guaranteed, and the placard itself is issued by Pinellas County.

Medical, surgical & pregnancy

Dr. Barry Schapiro, MD

Florida-licensed physician and surgeon.

Evaluates how a medical condition limits your ability to walk, including mobility limits from cardiac, pulmonary, neurological, or vision conditions, from recent surgery, and for expectant mothers. Certifies HSMV 83039 and HSMV 83040.

Do I have to live in Clearwater to use Provider Notes?+

No. You need to be physically located in Florida during your video visit. We serve Clearwater, the Tampa Bay area, and the rest of the state.

Where do I take my signed form?+

Clearwater is in Pinellas County, so your completed HSMV 83039 goes to the Pinellas County Tax Collector's office or a license plate (tag) agency in your county. Check your local office for locations and any requirements before you go. Many locations also accept applications by mail.

Is approval guaranteed if I pay?+

No. The fee covers the evaluation, not the outcome. Your doctor makes a clinical decision during the visit, and certification is not guaranteed. Our doctors are paid the same whether they certify you or not.

How much does it cost?+

A flat $119, charged 24 hours before your visit. Nothing is charged when you book. The eligibility check beforehand is free and requires no card.

How soon can I be seen?+

Most visits are available within about three days of booking. You choose a time that works for you.
