Florida-licensed physician and surgeon.

Evaluates how a medical condition limits your ability to walk, including mobility limits from cardiac, pulmonary, neurological, or vision conditions, from recent surgery, and for expectant mothers. Certifies HSMV 83039 and HSMV 83040.

Do I have to live in Solivita to use Provider Notes?+

No. You need to be physically located in Florida during your video visit. We serve Solivita, Central Florida, and the rest of the state.

Where do I take my signed form?+

Solivita is in Polk County, so your completed HSMV 83039 goes to the Polk County Tax Collector's office or a license plate (tag) agency in your county. Check your local office for locations and any requirements before you go. Many locations also accept applications by mail.

Is approval guaranteed if I pay?+
