Disabled parking permit for arthritis and mobility conditions.

If arthritis, a back or joint condition, or another musculoskeletal problem limits how far you can walk, a Florida-licensed chiropractic physician can evaluate you by private video visit and, if you qualify, complete Florida's HSMV 83039 form. You submit the signed form to your county tax collector’s office or a license plate (tag) agency, who issues your placard. $119 one-time fee. Free eligibility check first, no card required.

  • Chronic back, hip, or knee conditions that limit your range without resting
  • Musculoskeletal conditions that affect walking more than 200 feet without stopping

Do I have to be in Florida?+

Yes. You must be physically located in Florida during your video visit. The permit is a Florida permit.

Can a chiropractor certify a Florida parking permit?+

Yes. Under Florida law, a licensed chiropractic physician can certify a disabled parking permit application for qualifying musculoskeletal conditions. Dr. Fishman is licensed in Florida and evaluates these cases by video.

What if my condition is more medical than musculoskeletal?+

The eligibility check routes you to Dr. Schapiro, MD, instead. You do not need to decide in advance.

How much does it cost?+
