Do I have to live in St. Petersburg to use Provider Notes?+

Where do I take my signed form?+

St. Petersburg is in Pinellas County, so your completed HSMV 83039 goes to the Pinellas County Tax Collector's office or a license plate (tag) agency in your county. Check your local office for locations and any requirements before you go. Many locations also accept applications by mail.

Is approval guaranteed if I pay?+
