Do I have to be in Florida?+

Yes. You must be physically located in Florida during your video visit. The permit is a Florida permit.

Why is this a different form?+

Florida uses a separate application, HSMV 83040, for the expectant mother permit. It is temporary and, by law, must be certified by a physician.

How do I get the physical permit?+

Your county tax collector office or a license plate (tag) agency issues the physical permit once you submit the signed form. Many locations also accept applications by mail.
